Description
This 3.25 credit‑hour home study continuing education training is designed for practicum students and early‑career clinicians who want their notes to be both clinically solid and protective of client privacy. This training focuses on ethical documentation practices that respect client autonomy, minimize unnecessary disclosure, and still meet legal, ethical, and insurance standards in a range of clinical settings. It is especially oriented toward work with marginalized communities who are disproportionately harmed when records over‑document identity and life history.
This course includes:
- 1 hour and 10 minute recorded video lecture (1.25 CE credit hours) including
- Information on the Golden Thread principle of clinical documentation
- A lecture on concrete strategies to write notes that are useful for treatment, defensible under review, and likely to expose clients to carceral, immigration, legal, or other structural harm
- Our 8-part strategy for effective clinical documentation
- Examples of protective, succinct, and appropriate clinical documentation
- Revision exercises
- Supplemental materials (2 CE credit hours) including

Learning objectives
By the end of this training, participants will be able to:
- Describe how ethical, client‑centered documentation protects privacy, autonomy, and safety.
- Use the “minimum necessary” standard to decide what belongs in assessments, treatment plans, and progress notes.
- Revise notes that over‑disclose sensitive identity and life details while keeping them clinically accurate.
- Document mental status and risk in concise, neutral language that supports sound clinical and ethical decision‑making.
- Show a clear “golden thread” from diagnosis and impairment to interventions, client response, and plan.
Continuing Education
Participants will receive 3.25 NBCC credit hours for completion of all required course modules, including a quiz. The quiz requires a score of 80% or better and may be attempted 3 times. Laura Baker is approved by the Nevada Board of Examiners for Marriage and Family Therapists and Clinical Professional Counselors to offer continuing education, and the Loom Collective is a National Board for Certified Counselors (NBCC) Approved Continuing Education Provider (ACEP™), ACEP No. 7795. Pricing
Our pay‑what‑you‑can model invites providers to select a fee ($30-150) based on income and access to resources. When people with more access to wealth pay more, it makes it possible for us to offer low‑cost and free options to those with less access.

Presenters
Laura Baker Rathore, MA, LMFT (they/them) is a Nevada‑licensed marriage and family therapist, educator, and organizer specializing in gender‑affirming, anti‑carceral care for 2SLGBTQIA+ communities. As Executive Director of Inclusive Counseling LLC (2023–2026), Laura founded and led a liberation‑oriented counseling agency, developed the Gender‑Affirming Care Certificate Program, and created ongoing continuing education programs for interdisciplinary providers. In this role, they established agency‑wide clinical documentation standards for an insurance‑contracted practice and trained clinicians on writing notes that are ethically grounded, privacy‑protective, and audit‑ready. They have designed and facilitated trainings on gender‑affirming care, anti‑carceral crisis response, and recognizing so‑called “conversion therapy” for universities, professional organizations, and community groups, and serve as co‑founder and director of The Loom Collective, a continuing education project focused on sociopolitical and ethical issues in queer, trans, and otherwise marginalized mental health.
